Requirements for Registration
of Pesticidal Devices

| To register
a pesticidal device in Indiana, you must provide the following items
for each pesticide product: |

| 1. |
Indiana
application for pesticide product registration; |
| 2. |
The complete brand
name of the pesticidal device; |
| 3. |
A complete copy
of the label/labeling for the pesticidal device, outer retail packaging,
and a statement of all claims to be made for it, including directions
for use; |
| 4. |
A complete copy
of all written, printed, or broadcast claims made for the device; |
| 5. |
Product efficacy
data: A full description of the scientific tests made and the results
of the tests upon which the pesticidal claims for the device are
based. Testimonials and anecdotal information are not acceptable
as scientific tests to support that the device performs as claimed; |
| 6. |
A schematic diagram
for the device; |
| 7. |
Two representative
units of the device for our inspection (non-returnable); |
| 8. |
A check for the
$170.00 registration fee payable to Indiana State Chemist. |
