Procedures
for submitting Programs/Workshops/Events for approval of
INDIANA CONTINUING CERTIFICATION HOURS |

| STEP 1: |
Complete
the Request for Continuing Certification
Hour (CCH) Approval Worksheet. Please read all the instructions
before submitting the worksheet.
This worksheet is now available
as an on-line fill-in form. You can visit the Adobe
website to
make sure you have the most
current version of the FREE Adobe Acrobat Reader installed.
This
form has been enabled so that the user can fill it out, save
it with the data intact and then e-mail it to Jill Davis as an attachment.

| NOTE: |
a) The training must be open to the public.
b) Any registration fees must be the same for all participants.
(An exception can be made for professional educational organizations claiming IRS 501(c) status or governmental organizations ).
c) The trainers must be from outside the immediate organization of the trainees.
d) The meeting space must be large enough to accomodate a reasonable number of participants outside the immediate organization providing the training. |

| STEP 2: |
Submit the completed
worksheet to OISC at least three (3) weeks prior
to the program. |

|
| STEP 3: |
Contact OISC after
two (2) weeks if you have not been contacted regarding the approval
or denial of your request. |
